First Session

Print and fill out our initial intake form to bring to your first appointment or you can email it directly to your therapist.

Payment

Payment is due at the time of service. We accept cash, check, debit and credit cards. If you are using a card, we will process your payment through a secure HIPAA protected application called Ivy Pay.

Appointment Fees

Individual intake (60 minutes): $225

Individual session (50 minutes): $200

Cancellation Policy

If you cancel your appointment within 24 hours of the scheduled time or fail to show up at the appointment without notice (“no-show”), excluding emergency situations, we will charge you for the full amount of the session.  Please be aware that most insurance companies will not provide reimbursement for cancellation fees and you will be personally responsible for the payment of any such fees.

Insurance

We are currently out of network with most insurance providers. If you would like to use your out-of-network insurance benefits, you will be asked to pay the full fee at the time of service and then you will be provided a superbill at the end of each month to file directly with your insurance company for reimbursement in accordance with your plan and benefits. A superbill does not guarantee insurance reimbursement.

Please contact your provider to better understand your specific benefits. When contacting your insurance company the following questions will be helpful:

  • Do I have out of network benefits for psychotherapy with a licensed psychologist? What are the benefits?

  • How many sessions or hours are covered, and for what period of time?

  • What percentage of the service fee is covered?

  • Do I need to submit any forms to use my out-of-network benefits?


You have the right to receive a “Good Faith Estimate” (GFE)

A GFE estimates how much your medical care will cost and health care providers are required to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

• If you would like a GFE, make sure your health care provider gives it to you in writing at least 1 business day before your medical service. You can also ask your health care provider, and any other provider you choose, for a GFE before you schedule an item or service.

• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
